SUBJECT: Bone marrow transplantation was first successfully carried out in the 1960s and was initially performed for the treatment of acute leukeamia. However, bone marrow transplantation is now performed for a wide range of conditions including inherited bone marrow disorders such as severe combined immunodeficiency, chronic granulomatous disease and osteopetrosis as well as malignant bone marrow disorders including leukemia and myeloma. Transplantation may take the form of whole marrow from an unrelated donor or stem cells mobilised from the patient''s own bone marrow. More recently transplants from cord blood have also been used as a source of haematopietic stem cells.NEED: In this field, a range of paediatric and adult bone marrow transplanations are performed for a wide variety of disorders. The underlying condition of the patients and the immuno suppression used in bone marrow transplantation predispose the patients to a wide range of infections. In addition, other complications may arise as a result of transplantation, particularly graft versus host disease which requires a careful balance between suppressing the condition and trying to reduce the risk of infection at the same time.The management of patients requiring bone marrow and stem cell transplantation involves the input of large numbers of health care professionals and there is now a need for a practical, concise manual to guide all those working in this field.
